We are recruiting additional full and part time District Nurses to work as part of the growing and developing Community Nursing Service covering the Eastbourne, Hailsham and Seaford localities.

We are seeking experienced nurses with the Specialist Practitioner District Nursing qualification who are able to demonstrable clinical, management and leadership qualities and apply their knowledge of current best practice and the emerging evidence base for district and community nursing.

You will be responsible for managing a clinical caseload and day to day co-ordination of a team of Staff Nurses, HCA’s and support staff to ensure seamless delivery of high quality, safe and effective clinical care and support. This will include receiving referrals, carrying out assessments, deciding the most appropriate pathway, liaising with other ESHT, Adult Social Care and partner providers.

Our busy and dynamic Community Nursing Services are central to the transformation agenda for Health and Social Care in East Sussex. You will actively participate alongside the Integrated Locality Team Manager, Lead Nurses and other Health and Social Care professionals in the continuing implementation and development of Integrated Locality Teams. You must be able to travel in-between sites independently in a timely manner.
